How to Complete Your Beneficiary Form
1. Personal Information
Complete your name, member ID, Social Security number, and current contact information.
2. Primary Beneficiaries
List your primary beneficiaries with full names, addresses, dates of birth, Social Security numbers, and relationship to you. Specify the percentage each beneficiary should receive.
3. Contingent Beneficiaries
List contingent (secondary) beneficiaries who will receive benefits if your primary beneficiaries predecease you.
4. Signature and Date
Sign and date the form. Your signature must be witnessed or notarized as required by the form.
